Presenting a new patient health questionnaire for depression and anxiety. This simple questionnaire is a great tool available in  many languages like spanish, arabic etc. 

The original PHQ tool, comprising 67 articles based on Prime MD, was developed more than a decade ago to help clinicians diagnose common mental disorders.
